Getting To Know You Form - To be completed for each student following registration

Click embedded form below to launch an online fillable PDF form.  Please complete and submit the form.  You will be asked to enter your name and email address after you submit the form.  You will then receive an auto generated e-mail request to verify that your e-mail is deliverable.  YOU MUST COMPLETE THIS STEP FOR YOUR FORM TO BE SUBMITTED.  You will receive a completed copy of your submission via e-mail and a copy will also be sent to Summer Theater Camp. 
Students must complete the Getting To Know You form below as soon as they are registered and the deposit has been paid.  The Getting To Know You form must be submitted by June 7, 2025.
